This is the form for sending shoulder boards in for BuSup's Buy Back program.
You must complete all fields on this form, print it and post it to the address on the form with your shoulder boards. If your boards pass inspection you will be given store credit to the value of 50% of the BuSup price of the boards.
This will allow us to sell the second hand boards on to other members at a discount.
